Despite containing many pockets of affluence, Collin County is home to more than 150,000 residents who do not have health insurance, leaving them highly vulnerable to adverse health effects.
Additionally, there are considerable gaps in patient education. Many do not know the best ways to manage their health and need community resources. Texas Health is uniquely positioned to join with the local community to improve the health throughout Collin County.
The Community Resource Center is a multi-faceted intervention that improves health outcomes, fosters community connectivity, and empowers and educates Collin County residents.
The Texas Health’s 2022-2025 Community Health Needs Assessment (CHNA) determines areas in North Texas that struggle with non-medical drivers of health. Overall, 50% of health influences are traced to Zip Codes.
- Zip Codes in Collin County identified by CHNA:
- 75069
- 75074
- 75407
- 75442
In 2023, Texas Health served 4,167 patients on Medicaid or without health insurance from the five Zip Codes surrounding Texas Health Allen.
With the Community Resource Center focusing on high-risk populations, a primary funding need for this program is to have the costs of patient care covered.
Through this project, Texas Health strives to enhance access to healthcare for individuals and families living in high-needs areas within Collin County. In addition to meeting immediate needs, we aim to educate, empower, and connect patients to community-based resources that will help them better manage their health throughout their lives.
