Accessing and Requesting Health Records in MyChart

Most of your patient information can be found in your Texas Health MyChart account.

For immediate access to your health information, select the link at right and follow the directions to login or sign up for a MyChart account.

Submit a Health, Imaging, or Billing Records Request Online

Texas Health has partnered with Versima to provide copies of your health information. If you would like to submit a health record request online, please click on the following link(s) below, which will take you to an authorization form in Verisma's system.

Please note the different categories for requests by a patient or by a patient representative. Medical records requests that meet all HIPAA and state requirements are usually available for release within three to five business days after completion.

  • How do I check the status of my request?
    You may check the status of your request by calling 1-855-681-8243. If you received a “Medical Records Request Received” letter from our disclosure management partner, Verisma, you may enter the Access Code from the letter into https://track.verisma.com to track your request and download records if applicable.

  • Can I come in person to pick up my health records?
    You can pick up copies of health records at one of our walk-in locations:

    Enter through the main entrance and take a right. Medical records is located on the left just past Pastoral Care.
    Hours: Monday thru Friday, 8:00 a.m. - 4:30 p.m.

  • Does it cost to get my records?
    Texas Health does not charge patients for a copy of their health records.

  • How do I send copies of my records to my doctor?
    Patients requesting records for another provider visit outside of Texas Health should have the new provider contact Health Information Management Disclosure Management at 1-855-681-8243 or fax request to 1-214-345-8811. Records will be sent directly to the provider.

  • How can I request a computer readable copy of my electronic health information (EHI)?
    Patients or their legal representatives can request an export of their electronic health information. These files are not designed to be read by humans. You will need an application that can read computer-readable data. To request a copy of your computer-readable export, submit a request in MyChart or by contacting THR Disclosure Management at 1-855-681-8243.
