Social Security
Parents may request a Social Security number for their newborn at the time of birth. The Social Security Administration will assign your newborn a number and mail the card directly to you.

Birth Certificates
After you deliver, your nurse will give you a Vital Statistics Worksheet to complete. Your nurse will give you a Birth Facts letter before you leave the hospital. This has all of the information about your baby’s birth. Please be sure to review this carefully before signing it. Check the spelling of all names and all birth details. We can make changes before we send this information to the Texas Department of State Health Services Vital Statistics.
You will not automatically receive a copy of the birth certificate. You will need to order a copy. There is a $23 fee. To get a copy of the birth certificate, you can order it online at www.texas.gov. You can also go to the local court building in the county you delivered in. The state works to process your baby’s birth certificate. How long it will take for you to get your baby’s birth certificate depends on the state’s current processing times.
Your baby’s information will only be sent to the social security office to give your baby a social security number if you allow the baby’s information to be released. You will do this by checking “yes” to this question on the Vital Statistics Worksheet. The social security office will send your baby’s social security card to the address you provided to the hospital.
When adding your baby to a health insurance plan, health insurance companies will accept the Birth Facts letter you get at the hospital.
